Unlike women's relatively abrupt hormonal transition at menopause, men experience a gradual hormonal decline that begins in the 30s and accelerates through the 40s and 50s. This process — sometimes called andropause or late-onset hypogonadism — is less dramatic than menopause but no less significant in its effects on health, vitality, and quality of life.

The Hormonal Changes in Men After 40

Testosterone Decline

Testosterone is the primary male sex hormone, and its decline is the central feature of male hormonal aging. After peaking in the late teens and early 20s, testosterone declines at approximately 1–2% per year from the 30s onward.

By age 40, many men have testosterone levels 15–20% below their peak. By 50, the decline is more pronounced. By 60–70, many men have testosterone levels that would be considered clinically low by any standard.

This decline affects:

  • Total testosterone — the overall amount in circulation
  • Free testosterone — the biologically active fraction (declines faster because SHBG increases with age)
  • Bioavailable testosterone — the fraction available to tissues

SHBG Increases

Sex hormone-binding globulin (SHBG) — a protein that binds testosterone and renders it biologically inactive — increases with age. This means that even when total testosterone appears normal, free and bioavailable testosterone may be significantly reduced.

This is why testing free testosterone (not just total) is essential for accurate assessment.

Estrogen Changes

Men produce estrogen through the conversion of testosterone by the enzyme aromatase — and aromatase activity increases with age and body fat. As a result, many middle-aged men have declining testosterone alongside rising estrogen, worsening the hormonal imbalance.

Elevated estrogen in men contributes to:

  • Gynecomastia (breast tissue development)
  • Increased fat storage (particularly in the chest and midsection)
  • Reduced libido
  • Mood changes

DHEA and Growth Hormone Decline

DHEA — a precursor to testosterone — declines steadily from the mid-20s. Growth hormone, which supports muscle maintenance, fat metabolism, and tissue repair, also declines significantly with age.

Symptoms of Low Testosterone After 40

Physical Symptoms

  • Reduced muscle mass and strength — despite maintained exercise
  • Increased body fat — particularly visceral and chest fat
  • Fatigue and low energy — persistent, not explained by sleep
  • Reduced exercise tolerance and recovery

Diagnosing Low Testosterone

Diagnosis requires both symptoms AND laboratory confirmation. Testing should include:

  • Total testosterone — morning draw (testosterone peaks in the morning)
  • Free testosterone — the biologically active fraction
  • SHBG — to interpret free testosterone
  • LH and FSH — to distinguish primary from secondary hypogonadism
  • Estradiol — to assess aromatization
  • Complete blood count — baseline before TRT
  • PSA — baseline before TRT in men over 40
  • Metabolic panel — liver function, lipids, glucose

Optimal vs. "normal" ranges: Standard lab reference ranges for testosterone are broad (typically 300–1000 ng/dL) and based on population averages that include many symptomatic men. Many men feel their best with testosterone in the upper third of the range (600–900 ng/dL). Symptom assessment alongside lab values is essential.

Treatment Options

Testosterone Replacement Therapy (TRT)

TRT is the most effective treatment for clinically low testosterone. Options include:

  • Intramuscular or subcutaneous injections — most cost-effective; typically weekly or twice-weekly
  • Topical gels or creams — convenient; risk of transfer to partners/children
  • Pellet implants — inserted subcutaneously every 3–6 months; consistent levels

TRT requires monitoring of testosterone levels, hematocrit, estradiol, and PSA. When properly managed, TRT has an excellent safety profile and produces significant improvements in energy, body composition, mood, libido, and cognitive function.

Lifestyle Optimization

Even without TRT, lifestyle interventions can meaningfully support testosterone:
