Women's hormonal health is complex — and standard hormone testing often provides an incomplete picture. A single TSH or estradiol measurement, taken at an arbitrary point in the menstrual cycle, can miss significant hormonal imbalances that are directly affecting how you feel.
Understanding what a comprehensive women's hormone evaluation should include — and when and how to test — is essential for getting meaningful answers.
Why Women's Hormone Testing Is Complicated
Women's hormones fluctuate dramatically throughout the menstrual cycle. Estradiol, progesterone, LH, and FSH all follow predictable patterns — but those patterns vary significantly between the follicular phase (days 1–14), ovulation, and the luteal phase (days 15–28).
Testing at the wrong time of the cycle can produce misleading results. A progesterone level drawn on day 5 of the cycle will be low — but that's normal. The same level drawn on day 21 would indicate a luteal phase defect.
For women in perimenopause or menopause, the cyclical pattern is disrupted or absent, and interpretation requires a different framework.
What a Comprehensive Women's Hormone Panel Should Include
Estradiol (E2)
The primary estrogen in premenopausal women. Estradiol drives the menstrual cycle, supports bone density, cardiovascular health, cognitive function, and mood. Low estradiol produces hot flashes, vaginal dryness, brain fog, and mood instability. Elevated estradiol (estrogen dominance) produces heavy periods, bloating, breast tenderness, and mood swings.
When to test: Day 3 of the menstrual cycle for baseline assessment; day 21 for luteal phase evaluation.
Progesterone
Produced by the corpus luteum after ovulation, progesterone prepares the uterus for implantation, supports pregnancy, and has calming, sleep-promoting effects. Low progesterone (luteal phase defect) is associated with anxiety, insomnia, PMS, irregular periods, and difficulty conceiving.
When to test: Day 21 of the cycle (7 days after presumed ovulation) to assess luteal phase adequacy.
Testosterone (Total and Free)
Women produce testosterone in the ovaries and adrenal glands. Optimal levels support libido, energy, muscle tone, mood, and cognitive function. Low testosterone in women produces fatigue, low libido, reduced motivation, and difficulty maintaining muscle mass.
When to test: Any time; not cycle-dependent.
DHEA-S
DHEA is an adrenal hormone that serves as a precursor to both estrogen and testosterone. It declines significantly with age and stress. Low DHEA-S is associated with fatigue, reduced libido, and impaired immune function.
FSH and LH
Follicle-stimulating hormone (FSH) and luteinizing hormone (LH) are pituitary hormones that regulate the menstrual cycle. Elevated FSH is a marker of diminished ovarian reserve and perimenopause. The LH:FSH ratio is relevant in PCOS assessment.
When to test: Day 3 of the cycle for ovarian reserve assessment.
Sex Hormone-Binding Globulin (SHBG)
SHBG is a protein that binds to sex hormones, making them unavailable to tissues. High SHBG (common with oral contraceptive use, thyroid dysfunction, and liver conditions) reduces the amount of free, biologically active estradiol and testosterone — even when total levels appear normal.
Thyroid Panel
Thyroid dysfunction is significantly more common in women and profoundly affects hormonal balance. A comprehensive thyroid panel (TSH, free T3, free T4, reverse T3, TPO antibodies) should be part of every women's hormone evaluation.
Cortisol
Chronic stress and cortisol dysregulation directly impair sex hormone production and balance. A 4-point salivary cortisol test maps the full diurnal pattern and is more informative than a single blood cortisol measurement.
Insulin and Glucose
Insulin resistance is a major driver of hormonal imbalance in women — particularly in PCOS. Fasting insulin and glucose should be included in any comprehensive hormonal evaluation.
Interpreting Results: "Normal" Is Not the Same as "Optimal"
Standard lab reference ranges are based on population averages — they tell you whether you're within the range of the general population, not whether your levels are optimal for your health and wellbeing.
